Diffusion tensor imaging (DTI) has become a valuable tool in brain tumor surgery, offering insight into the complex microstructural architecture of white matter regions. This thesis explores the key role of DTI in improving preoperative planning and intraoperative navigation, promoting the preservation of critical neural pathways, and minimizing postoperative neurological deficits. By providing neurosurgeons with detailed information about tumor infiltration and its relationship with important brain regions, DTI facilitates improved surgical strategies, resulting in improved patient outcomes and quality of life.

PREDICTING THE CONSEQUENCES OF CLINICAL, IMMUNOLOGICAL AND IMMUNOCHEMICAL RESULTS OF RELATED KIDNEY TRANSPLANTATION

The article describes the effectiveness of the acetylation phenotype in clinical use, taking into account the identified relationships between the acetylator status of the donor and recipient, and graft function, the risk of rejection and the choice of immunosuppressive protocol, in terms of predicting the results of related kidney transplantation. The developed method for predicting possible complications after KT taking into account the acetylation phenotype, CYP3A5 genotype and glomerular filtration rate allowed us to classify the risk as low, medium or high.

METHOD FOR ELIMINATING THE RESIDUAL CAVITY AFTER HEPATIC ECHINOCOCCECTOMY

Despite significant advances in the surgical treatment of liver echinococcosis (LE), many aspects of this condition, which potentially have a decisive influence on postoperative outcomes and the quality of life of patients, are far from being definitively resolved and remain subjects of further research and discussion. The optimization of tactical and technical aspects of LE surgical treatment is one of the most debated and unresolved areas in surgery. A primary concern remains the recurrence of LE, indicating the need for refining indications for existing treatment options, methods of cavity elimination, and treatment post-echinococcectomy, as well as the development of prognostic scales considering risk factors for different morphological forms of echinococcal cysts. The article highlights current views on the surgical treatment of LE and controversial issues in selecting the optimal strategy.

RESECTION TECHNIQUES IN RELATED LIVER TRANSPLANTATION

Relevance. Transplantation is the only radical treatment for end-stage liver disease. Improving the results of liver transplantation directly depends on the preparation of recipients, careful selection of donors, surgical technique and methods of rehabilitation in the postoperative period. Despite the positive results achieved in the survival of recipients after transplantation, the complication rate is still high, which requires further research in this direction. The purpose of the study: to analyze the results of the initial stage of the introduction of related liver transplantation in the Republic of Uzbekistan. Research materials. For the period from February 12, 2018 to February 2024, 96 related transplantations of the right lobe of the liver were performed in patients with decompensated liver failure of various etiologies. Research results. The postoperative period in recipients passed without complications in 55 cases, which amounted to 57.3%, in the remaining 41 patients (42.7%), one or more complications were noted after the intervention. Thus, among 41 patients, the development of 72 different complications (from 1 to 3 complications) was noted. At the same time, complications that occurred in 21 patients (21.9%) in the early postoperative period required relaparotomy. Among 41 patients, 15 patients (15.7%) died in the early postoperative period, in the long-term period (3 years after transplantation) there was another 1 death in a patient with chronic rejection against the background of refusal to take immunosuppressive drugs. In general, among the recipients operated at the center for the period from 2018 to 2024, the nearest satisfactory results were obtained in 84.4%, while in the long-term period this figure was 79.2% Summary. The analysis showed that the survival rates of grafts and recipients to a large extent depend on the characteristics of surgical tactics, nosology, and the severity of the initial state of the recipient. At the initial stages of the implementation of the program, the so-called “center effect” is essential.
