DIFFERENTIAL APPROACH TO ENDOVASCULAR TREATMENT OF CRITICAL LOWER LIMB ISCHEMIA

Abstract. In the world of last decades of the 20th century and at the beginning of the 21st century, specialists in the treatment of patients with diabetes (DM) pay attention to the diabetic foot syndrome (DFS) with critical ischemia of the lower extremities and diabetes and its complications combine "pathological processes of the microcirculatory, peripheral nervous systems, bone and articular apparatus of the foot, which pose a direct threat or development of ulcerative necrotic processes and gangrene of the foot. According to some epidemiological studies, the incidence of "peripheral artery disease (PAD) in patients with diabetes is from 10 to 40%, and in the presence of foot ulcers reaches 50%". In Uzbekistan, CINC was diagnosed in more than one hundred thousand people. Vascular atherosclerosis is the cause of chronic lower limb ischemia (CLLI) in 80-90% of cases. Especially often this pathology is observed in people over 60 years of age.

THE ROLE AND TREATMENT OF TESTOSTERONE IN MEN WITH TYPE 2 DIABETES

Abstract. Epidemiological studies consistently demonstrate that low serum testosterone is not only common in men with established type 2 diabetes, but also predicts future diabetes risks and increased mortality. Preclinical studies report plausible mechanisms by which low testosterone may mediate dysglycemia. Treatment with exogenous testosterone consistently reduces fat mass, increases muscle mass, and improves insulin resistance. Large, well-designed clinical trials are needed, given that the benefit/risk balance of testosterone treatment is not well understood.

Abstract. Computerized hernioabdominometry (CTGA) was performed to determine the size of the hernial orifice, the volume of the contents of the hernial sac, as well as to preliminarily determine the method of hernioallo- and abdominoplasty. This method was performed in 58 (86.6%) patients with ventral hernias and morbid obesity. The obtained forms of violation of the topography of the anterior abdominal wall made it possible to develop a rational preparation of the patient before the operation, as well as to choose the optimal method of hernioplasty individually for each patient.

FEATURES OF HERNIOPLASTY AND ABDOMINOPLASTY IN PATIENTS WITH POSTOPERATIVE VENTRAL HERNIA AND ABDOMINOPTOSIS

Abstract. An analysis of the results of a comprehensive examination and surgical treatment of 121 patients with ventral hernia and concomitant morbid obesity using a differentiated approach to the choice of the method of hernioplasty and abdominoplasty is presented. Improvement of the technical aspects of non-stretch methods of hernia alloplasty with implantation of the endoprosthesis using the combined onlay + sublay method in combination with abdominoplasty made it possible to level the recurrence of the disease (in the comparison group, recurrence was 12.5%). The proposed algorithm for adequate preoperative preparation and choice of the method of hernia allo- and abdominoplasty in patients with abdominal hernias and abdominoptosis made it possible to reduce the incidence of postoperative complications from 14.8% to 8.9% (p=0.045), including wound complications from 11.5% to 4.5%.

INFLUENCE OF MICROBIOLOGICAL FERTILIZER ON COTTON UNDER CONDITIONS OF TYPICAL GRAY SOILS

Abstract. This article presents information about the effect of microbiological fertilizer Baikal EM-1 on the growth and development of cotton. The use of microbiological fertilizer Baikal-EM-1 in combination with nitrogen, phosphorus and potash fertilizers on cotton, as well as studies on cotton without mineral fertilizers, using microbiological fertilizer at different times and methods are noted. It is known that the application of the microbiological fertilizer Baikal EM-1 against the background of N-200R-140K-100 kg/ha in the conditions of typical old-irrigated gray soils improves the agrochemical properties of the soil and the absorption of nutrients by plants. Under the conditions of typical gray soils, irrigated for a long time, scientific data were obtained on the effect of the microbiological fertilizer "Baikal-EM-1" with and without mineral fertilizers on the agrochemical properties of soil and plants, cotton yield, and technological properties of the fiber. The influence of microbiological fertilizer against the background of mineral fertilizers on the change in the amount of nutrients in cotton pieces and their absorption was established. The introduction of microbiological fertilizer Baikal EM-1 N-200R-140K-100 kg/ha under the conditions of old-irrigated typical gray soils has a positive effect on the growth and development of cotton compared to the control variant (option 1), the height of the cotton bush is 1.4 cm, the yield of branches has increased up to 1.2 pcs., the number of bolls increased to 1.1-1.4 pcs., which had a positive effect on the yield and quality of cotton.
